Patricia Ann Smith

Patricia "Pat" Ann Smith, age 88, of Rockford, passed away on Sunday, December 21, 2025.

Pat was born on April 13, 1937, in Rockford, IL, the daughter of Joseph and Ermenia “Marge” (Pozzani) Lozdoski. She graduated from West High School, Class of 1955 and then from St. Anthony School of Nursing in 1958. Pat married George Alex Smith in 1958. Pat honorably served her community with great care and passion as a registered professional nurse in her lifelong career, mainly employed by John E. Tillis, MD. She was a member of the St. Anthony Alumni Association and the Belvidere Moose Lodge, and a longtime volunteer at OSF St. Anthony Medical Center and the Belvidere Moose Lodge.

Deeply committed to serving others, she often brought her granddaughter along with her, teaching her the value of community and compassion through her example. In recognition of her dedication, Pat was honored as the Lifescape Senior of the Year in 2001.

Surviving relatives include her grandchildren, Andrew Robert Hade, Allison (Phil) Forsythe; sister, Shirley Ann Johnson; niece, Julie Earp and numerous nieces, nephews and cousins. Patricia is predeceased by her parents; her daughter, Katherine S. Hade; sister, Dolores Staskauskas and 2 brothers, Wayne and Richard Lozdoski.

Memorials may be made in Patricia's name to St. Jude Children's Research Hospital.

A funeral service will be held at 10:30 a.m. on Friday, January 2, 2026, with visitation from 10:00 a.m. until the time of service located at Fitzgerald Funeral Home & Crematory, 1860 S. Mulford Rd. Rockford, IL 61108. Final resting place will be in Willwood Burial Park, Rockford, IL.
