Chicago—Mr. Theodore “Ted” Kozaritz, 63 died Wednesday, June 8, 2016 at Amberwood Care Centre in Rockford, IL. Born September 1, 1952 in Chicago, Illinois to Joseph and Dolores (Gapinski) Kozaritz.
Survivors: Siblings, Gerald (Pat) Kozaritz, Mary Dee (John) Conroy, Colette (Pete) Rasiarmos, Candice (Roger) Kocolowski, William Kozaritz and Cynthia (Chuck) Lindenmuth; Numerous nieces and nephews. Predeceased by his parents Joseph and Dolores and brother Joe.
In lieu of flowers memorials may be made to Parkinsons Disease Research Parkinson's Disease Foundation, 1359 Broadway, Suite 1509, New York, NY 10018. Cremation rites have been accorded. All services private.
